This study utilized the descriptive-correlational research design. The respondents of this study were sixty-three (63) parents/guardians and the Grade 11 students at Bentung-Sulit National High School for the School Year 2020-2021. The researchers used the convenient sampling technique in selecting the respondents. The researchers utilized a modified research questionnaire and this was given to the respondents through online survey using the google forms. The data were collected, analyzed and interpreted using mean, and the Pearson-Product Moment Correlation to find the significant relationship between the two variables. All tests were done at 0.05 level of significance. It was found out that the parents and students had high anxieties on Covid-19 vaccine. They had doubts on the effectiveness of the Covid-19 vaccine. Meanwhile, it was revealed that there was a very high significant relationship between the parents’ and students’ anxieties on Covid-19 vaccine. This implied that the student’s decision towards covid-19 vaccination was affected by the decisions of their parents. Thus, it was recommended that the school and LGU should organize seminars/orientations/symposium to the parents and students to explain further the importance and benefits of Covid-19 vaccine.
