This paper aims to determine grade 5 learners’ level of performance in Mathematics and the factors affecting such performance. The results of the First Periodical Test for S.Y. 2018-2019 discloses that learners have low performance in Mathematics. Further, the results reveal that grade 5 learners found difficulty in solving routine and non-routine problems involving addition and/or subtraction of fractions using appropriate problem solving strategies and tools and in creating problems (with reasonable answers) involving addition and/or subtraction of fractions using appropriate problem solving strategies. Using the qualitative and quantitative methods of research, the researchers find out that the major contributory factors to learners’ poor performance in Mathematics are adequacy of learners’ materials, non-mastery of pre-requisite skills, the kind/quality of activities/practices provided in the learning materials, and learners’ poor comprehension skills. The findings of the study serve as guide in formulating and crafting intervention materials and activities that will help enhance learners’ performance in Mathematics.
